A Long awaited Holiday

Gary Duckworth is the name he always was proud of. Listening to the alarm he opened his eyes, waking up to the bright sunny day ahead. He was more than happy to have the day off today as it was a business day for most of the world around. It was such an inner twist in which he was assigned to meet Miss. Siena for an interview. He has set everything ready at the office for him to be away from the desk. Gary came to the living room, his hands ached very energetic from the work out he had for his biceps last evening. Spending time at the gym was something he always longed for. He just flexed his biceps to have a look at them in the mirror and was extremely happy with the huge budges. He enjoyed the caresses of admiring curious eyes along with words and phrases of admiration as he move about in the crowed. He went to the phone and dialed the number of Siena; he needed to conform once again that she would be available at 9. 30 in the morning at parker’s office in down town Manhattan. Parker’s office is in one World Trade Centre on 95th floor. He has been there quiet few times, always with Siena. The thought of Siena made him glad and gave him a sense of fulfillment. The relationship which has started as just a friendship very soon found itself with a lot of new meanings. Very soon both couldn’t help seeing each other and started to miss each other more than anything. As their romantic life grew Siena grew towards being an organization than a person in the music industry. Gary chose the best attire he always thought was the white shirt with black pants and the suite. He took the train to down town Manhattan. The train ride was rather calm and easy. Very few souls were found seated in the train as it was already past the rush hour. In about twenty minutes he would be at John street subway station and can walk over to the world trade centre. 2 He was walking out of the John street station when he thought of the day ahead though it was an official meeting the rest of the day e is going to be with Siena, he was going to make sure that this is going to be a holiday in all its sense. He was walking up the stairs when he noticed that all the people out at the street were looking upward in shock or rather in curiosity. He came out and found one of the twine towers were on fire. The first thing came to his mind was that there might be an arsenal accident in one of the offices. He did not want to waist the time standing on the ground and watching the fire. He wanted to go fast and meet Siena as soon as possible. She might be worried. He heard some one saying ‘might have been an accident’; it made no sense again to ask what is it. As he walked, there was the big bang, the earth shook, Gary just could not realize what was it but he could recognize the roar of people following the big noise and he could make out some thing from the crowed that ‘it is on purpose’†¦Ã¢â‚¬ ¦ ‘some on is doing it’†¦Ã¢â‚¬ ¦ ‘they are targeting us’. Who are they? Gary never knew. It was not the safest and joyful downtown any more. Gary in no time found himself in a Warfield of innocent civilians running for life not knowing in the concrete jungles where to hide. Gary became one of them in number, ran for his life and stumped up on some thing soft†¦wasn’t it a girl fallen on the street †¦Ã¢â‚¬ ¦. ho knows. Run, run and run with the crowds. The objects with fire are flowing from the towers, wind taking them all around even worse that is targeting us? What are they doing? Do they want me to be dead? Who do the want to terminate? What are they up to? Will there be yet another explosion and one of these high raisers may fall on me too? My God where is Siena? He looked up to the burning towers and saw the birds flying out from there but those black and white birds did not fly off. Rather just shot themselves down and splashed on the floor in red. Question-Answer Relationships, or QAR, is a reading comprehension strategy developed to aid in the approach that students take when reading texts and answering questions about that text. Students learn to categorize types of questions which in turn help them know where to find information. It encourages students to be active, strategic readers of texts. QAR outlines where information can be found â€Å"In the Text† or â€Å"In my Head. † It then breaks down the actual question-answer relationships into four types: Right There, Think and Search, Author and Me, and On My Own. (Fisher, D. , Brozo, W. G. , Frey, N. , & Ivey, G, 2011, pg. 81) STEP-BY-STEP and EXAMPLE Chosen text: Frog and Toad Together, by Arnold Lobel 1. Hook/Engagement–Begin by reviewing what students have already learned about how to ask questions as a way to understand the meaning of texts. For example using this reading asks them to talk about the kinds of questions they can ask before, during, and after reading. Next, introduce the idea that there are two kinds of questions you can ask about texts. Explain to students that an â€Å"In the Text† question is a question that students can find the answer to by looking in the book that they are reading. An â€Å"In My Head† question is a question that requires students to think about what their own knowledge is to answer the question. Review a book that you have recently read aloud with students. Write the example below on a piece of chart paper or on the blackboard. Choose a few â€Å"In the Text† and â€Å"In My Head† questions about the book that obviously belong to one category or the other, and have students tell you in which column to write the question. When you give students a literal question, have them show you where they found the answer in the book. When you ask them an â€Å"In My Head† question, go through the book with them and show them that they couldn’t find the answer in the book. Have them give answers to the â€Å"In My Head† questions and explain how they answered them ( thinking about what they have learned that is not in the book). Psychology †Nature/Nurture Debate Essay

â€Å"Outline and comment on the two schools of thought involved in the study of the nature-nurture debate in psychology. Explain, using examples, why this debate gives rise to so much controversy.†Ã‚  The debate concerning the influence of nature and nurture (or heredity and environment) on human behaviour is one of the longest running, and most controversial, both inside and outside psychology. It deals with some of the most fundamental questions that human beings ask about themselves, such as ‘How do we come to be the way we are?’ and ‘What makes us develop in the way we do?’ (Gross 2005, P.900) There are three sides to the debate: on the nature side are the nativists or ethologists who believe that children develop almost entirely as a result of genetic influences, with their environment having little effect; on the nurture side are the behaviourists or empiricists who believe people are born as a blank slate which is ‘filled-in’ over a lifetime through learning and experience; and in the middle are the interactionists who, hence the name, believe children develop as a result of an interaction between biology and environment. I will now look at the arguments in more detail. Genetic transmission is the way we acquire characteristics through inheritance. Each cell in the body contains a nucleus, which contains a substance called DNA. The main role of DNA molecules is the long-term storage of information. DNA is organised into long strands called chromosomes, and each chromosome is made up of thousands of genes. Genes are the basic unit of hereditary transmission and direct the way that growth and development happen within a plant or animal. Just after an animal is conceived, it is made up of a tiny group of cells. As these grow and divide, each gene acts as a code or set of instructions for making a particular protein. These proteins control the cell’s internal chemistry and tell the cell what to do, giving the organism particular characteristics and determining the way its body functions. We inherit 23 pairs of chromosomes from our parents, 46 in all, half from our Mother and the other half from our Father. They combine to produce all the information an embryo needs to develop biologically. Since we inherit particular chromosomes through the egg and sperm, we also inherit the particular characteristics coded for by the genes on those chromosomes. Arnold Gesell, a pioneer of developmental psychology, was an extreme nativist. He believed all individuals pass through the same genetically programmed series of changes, with the instructions for these changes being passed on at the moment of conception. â€Å"Gesell was mainly concerned with infants’ psychomotor development (such a grasping and other manipulative skills), and locomotion (such as crawling and walking)† (Gross 2005, P.901). Gesell established a research institute devoted to identifying ‘normal’ ages for a wide variety of behaviors and characteristics; he used a motion picture camera to film thousands of children in various stages of development. This genetically programmed series of changes is called maturation. It is important to look at maturation as we try to understand genetic influences on behaviour. Some genetic influences are obvious at birth such as hereditary illnesses or abnormalities such as Down’s syndrome, but the things we inherit don’t necessarily show up all at once. â€Å"The physiological changes which take place during puberty, for example, arise because of genes that are present at conception, but they only happen when the body is mature enough for them to take place. In the same way, certain forms of behaviour may only emerge once the individual is mature enough† (Hayes and Orrell 1998, P.7). In 1938 Lorenz and Tinbergen put forward four characteristics to identify directly inherited behaviour in animals. These are: stereotyped behaviour, which always occurs in the same way because behaviour which is directly caused by genetic influence can’t be affected by the environment; species-specific behaviour, because each species has its own genetic make-up the behaviour should differ to that of other species; the behaviour should appear in animals raised in isolation, because if it is truly inherited there should be no need to learn it; and the behaviour should appear complete even if the animal has not had chance to learn it.

Violence serves as a catalyst of this conflict escalation, it attracts attention and sympathy, but it also creates a certain image of the colony, which plays against its successful chances to save their culture, identity, and dignity.

Why Is HRT Prescribed For Menopausal Women This dissertation to answer the research question (Why HRT is prescribed for menopausal women despite the risk of breast cancer?) was carried out as mentioned in the methodology section using the literature review methodology. The selected studies were appraised with the critical appraisal tools by the Public health resource Unit. The methodology of literature review had well assisted to review all the studies in the perspective of the other and divulge meaningful evidence and information which in a single study capacity would have been otherwise unnoticed. The Critical appraisal tools have assisted to review all the studies comprehensively to ensure there validity, reliability and applicability so that the result could be generalized to all the menopausal women population. The studies which have been reviewed were conducted in various settings with participants from all types of study population hence the results are a wholesome reunion of the existing predisposing factors with the associated risk of HRT in the incidence of breast cancer. ANSWER TO THE RESEARCH QUESTION Every doctor has the relief of the patient of utmost importance while prescribing a treatment and every treatment is meant to bring relief. And treatment has dose schedules, administration guidelines and perhaps side-effects which are generally explained by the doctor. But unlike other medicines there is a lot of unrest around the use of HRT though the associated risk when evaluated with other factors is not high. Keeping the results and conclusions derived from these studies in the present and elements for further research in front, it is seen that HRT does have benefits. These associated benefits with use of HRT according to this study are: 1) Use of HRT decreases the risk of death overall (Sellers et.al,1997) (Sener et.al, 2009) and is associated with recurrence, metastasis-free survival and better overall and disease-free survival than HRT nonusers in the univariate analysis irrespective of the start of menopause (Sener et.al , 2009) (Bonneir et.al, 1998) 2) HRT does not increase the risk of breast cancer when administered to women for whom other risk factors have been excluded (Tzingounis et.al, 1996). 2) There is an inverse relationship between HRT and mortality due to coronary heart disease, stroke and cancers other than breast (Sellers et.al, 1997) 4) Use of HRT gives fewer locally advanced cancers and smaller and better-differentiated cancers compared to non-users (Bonneir et.al, 1998). 5) HRT is a favourable prognostic factor for breast cancer. (Bonneir et.al, 1998) 6) Use of HRT counteracts the increased incidence of breast cancer with the lower incidence of other tumors. (Olsson et.al, 2001) 7) Long term HRT use has a favorable effect against colorectal and endometrial cancer. (Writing Group for the Womens Health Initiative Investigators, 2002) (Corrao et.al, 2008). 8) Use of trans-dermal HRT compared to the oral use of HRT is associated with lesser risk of breast cancer (Million women study collaborators, 2003) (Corrao et.al, 2008) which is presumed by the WHI, 2002. However these benefits depend on many other inter-connected factors of duration of use, age of the menopausal woman, past personal history of HRT used, family history related to breast cancer, dose of the HRT administered and type of HRT regime prescribed. These same factors affect the element of risk as well in the following way. 1) Risk of breast cancer may be increased with HRT use for duration of 5 years or less in women with family history of breast cancer (Sellers et.al,1997) (Olsson et.al, 2001). 2) Increased risk of breast cancer after prolonged use of HRT (Olsson et.al, 2001) (Sellers et.al, 1997) (Corrao et.al, 2008) (Sellers et.al, 1997) (Olsson et.al, 2001). This risk increased with increasing duration of use and decreased with time and reached at baseline with 5 years time. (Ewertz et.al, 2005) (Million women study collaborators, 2003) 3) Higher risk in current users than in past users which was greater for combined therapy than for other oral types of HRT (Million women study collaborators, 2003). 4) Women who used combined estrogen and progestin HRT regime are at increased risk of breast cancer (Stahlberg et.al, 2004) (Million women study collaborators, 2003) and Oral HRT use for long term had a higher risk of breast cancer than trans-dermal use of HRT (Corrao et.al, 2008). 5) Increased risk of breast cancer with current use of HRT in women of 50 years of age and above which increased with increased use. No increased risk in women between the ages of 40-49 (Ewertz et.al, 2005) (Million women study collaborators, 2003). 6) The risk of breast cancer increased in women who were current HRT users and had used OC in the past. (Lund et.al, 2007) 7) HRT users developed breast cancer at a younger age than non HRT users (Sener et.al, 2009). According to the synopsis of the issues and factors derived it can be concluded that there are benefits associated with use of HRT which mutually depend on the risk factors. And this is the reason why HRT is prescribed for menopausal women despite the risk of breast cancer. And these will again depend on gynecologist judgment and patients level of knowledge and awareness (2006). RECOMMENDATIONS To bring the optimal benefit with minimally associated risk, HRT could be prescribed to menopausal women for lesser duration which the studies present as 5 years. This duration which could be disputable and the age of the women also has to be considered with the link of different effects of HRT in different age groups would again depend on the individual gynecologists judgment and the patients compliance for regular follow up and mammographic screening. An attempt has been made to clear the otherwise existing dilemma of prescribing HRT or not but this again depends on many other factors which are most importantly the patients follow up and the doctors perception of the associated prescribing strategies. However more research is needed as within the limitations of this dissertation though it is possible to conclude that there are benefits associated with the use of HRT despite the risk of breast cancer; it is not possible to chalk out an effective prescribing strategy. And to bring any considerable changes a prescribing strategy and better patient compliance for follow-up would be needed. LESSONS LEARNT These total results, conclusions and opinions from the studies have presented some principle elements which could guide through the dilemma of prescribing HRT to yet another menopausal woman. 1) It is evident that duration has a role to play in the increase of the foresaid risk of breast cancer and that there is no risk with the use of HRT in the past. 2) A lot would also depend on the gynecologist or the physicians prescribing principles. It would be needed by them to follow some guiding factors like evaluating high-risk and low-risk women, family history and previous history of Oral Contraceptives which would vary for each patient. 3) A regular follow-up and timely check up as mentioned would help to catch otherwise unnoticeable breast changes. 4) Another factor which needs to be pursued for future research and studies is the type of HRT regimes. In routine HRT is prescribed orally hence less is known about other methods of administration which could perhaps assist in reducing the risk. 5) It would also be helpful to find if there is a link with the BMI and use of HRT. Generally obesity is linked to many diseases and unhealthy conditions and this would help to categorize women into high or low risk group. This dissertation has tried to bring up some associating factors which could help to extract the optimal benefits with lesser risk. This systematic approach has helped conduct this dissertation so as to answer the research question. However research in healthcare is never ending and is expanding every day (Aveyard, 2010). New studies with various designs are being conducted around this topic as I write my dissertation which will bring in more new opinions and evidences which will form base for yet another dissertation or study tomorrow to help develop a successful prescribing strategy.

Information Asymmetry :- Another factor that influences the budgetary slack building behaviour r is the degree of information asymmetry. Information asymmetry arises when the subordinate managers possess private information regarding divisions productivity, effort level, and resource requirements that is not available to the central management. This private information can be used by the subordinates to maximize their self-interest especially when the central management cannot observe the subordinate actions. Subordinates have incentives to misrepresent their private information in order to set easy to achieve budget targets . NEGATIVE SIDE OF BUDGETARY SLACK :- Even though there are positives and negatives for budgetary slack, the negative effects outweigh positive effects .Some of the negative effects are the following:- 1)During good years managers tend to depend on budgetary slack and at bad years these managers tend to convert budgetary slack to profit. That is if a manager behind the profit for the year, the manager will rely on the budgetary slack built into the budget to make the profit goal anyway. But if manager is ahead of profit for the year then the manager will use up the additional money left in the budget because of the budgetary slack built into at the beginning of the year. Thus the managers will rarely, if ever exceed the goals although they will almost always meet their goals but the meeting of the goals will not be real since the goals are not real due to budgetary slack. 2) A deliberately understated sales budget might have serious consequences in planning other activities. For instance, production might be too low; the advertising program and distribution expense budgets may be planned incorrectly; the cash flow plans might be inaccurate. WAYS TO PREVENT BUDGETARY SLACK:- Evaluation system:- Organizations can reduce budgetary slack when they stop using budget as a negative evaluative tool. For instance, instead of criticizing subordinates every time a budgeted target is not achieved, if the subordinates are allowed some discretion to exceed costs when unavoidable, there will be a lesser tendency to create budgetary slack (Smith et al, 2008, p. 460). Truth- inducing pay scheme:- A problem occurs in participative budgeting when a subordinate has private information about factors that influence his or her performance and the pay scheme is budget or standard- based. The employee has an advantage to keep away this information from superiors so that a relatively easy standard is set, thereby creating slack. To eliminate this problem, research proposes a truth- inducing pay scheme to motivate subordinates to accurately communicate private information and maximize performance, thereby reducing budgetary slack (Waller, 2002). Budgetary Participation :-Is an ideal budgetary process. Most companies deviate from this ideal budgetary process.
